What's the Deal With X-Rays?

One of the biggest breakthroughs in dentistry, and radiology as a whole, was the advent of digital radiographs. Digital radiographs not only instantaneously processed an image but also reduced time, man power, and exposure to harmful chemicals when processing traditional films. It even reduced the amount of radiation exposure to our patients.

X-rays, or more formally, radiographs, are essential for dentists to diagnose several things like: the amount of bone around your teeth, decay in between your teeth that can't be seen clinically, and if any infection is brewing at the tip of your tooth root. We also use the panoramic radiograph, or the “big head scan” x-ray, to check your jaw bones, sinus, and even neck to make sure nothing menacing is brewing where we can’t see.

How much radiation am I getting?

An individual intra-oral dental radiograph emits 0.005 millisieverts (mSv) of radiation

Yearly radon exposure from your home is 7.50 mSv, 1,500x’s more radiation than a single dental radiograph 
 
A single full body CT scan is 10 mSv, 2,000x’s more radiation than a single dental radiograph 

Even a cross country plane ride is .025 mSV, 12’x more than a single dental radiograph
 
Even though dental radiographs are relatively safe and risks are very low, we still take maximum precaution when prescribing how many radiographs to take for your exam. We practice ALARA: As Low As Reasonably Achievable, which means we strive minimize exposure to radiation well below actual dose limits. Here at McAdams Dental, we use the lowest possible exposure which reduces the amount of radiation exposure 12x’s less than traditional film radiographs.
 
Not every one gets the same number of radiographs, some people need more, some need less. We do our best to individually manage risks and benefits when prescribing the amount of radiographs for our patients. 
 
In short, dental radiographs emit low radiation and are necessary to help us make the most accurate diagnosis to keep your mouth as healthy as possible.

Tooth Truth from Dr. McAdams

​You’re doing everything your dentist tells you to do, brushing twice daily even on a prescription tooth paste and you’ve done great with flossing daily. But you still got a cavity at your check up, what happened?

​Well, its complicated and there are many factors, but to keep it concise, its an issue with pH balance or how acidic your mouth is and how much bacteria you have in your mouth. 
We all know sugar is bad for your teeth, there’s no need to hammer that home but, sugar can be in foods you didn’t even realize and its important to consider the acidity of the drinks you ingest.

Many of the drinks we enjoy are unfortunately acidic. Coffee, Kombucha, Sugar Free Diet Sodas, even simple Carbonated Water have very low pH levels meaning they are HIGHLY acidic. Sipping on these drinks for long periods of time throughout the day creates a consistent acid bath which bacteria thrive in which eventually cause tooth decay. Also, if you’re drinking sugary and acidic beverages like Energy Drinks, Sodas, Orange Juice, and Sweet Tea throughout the day, you’re not only exposing your teeth to a constant acid bath but also a constant sugar bath, the worst combination.
 

Also…

Bacteria, like us, love carbs, which are sugars. 

We know of course that candy is not healthy for your teeth, but your carbs don't necessarily need to be sweet to cause tooth decay. Snacking, especially on chips and bread can easily stick to teeth and cause increased plaque formation which is a bacteria party. 

So its important to keep that in mind when you’re sitting in your office or at home in the afternoon thinking about sipping on your fifth cup of coffee or your third helping of Chex mix, to think about which type of “bath” your teeth are taking and what kind of “party” the bacteria are having.  

But remember, life is about balance, so treat yourself everyone once in awhile...

...and brush twice daily.

Dentistry 101 with Dr. Gibbons

In todays blog, Dr. Gibbons will review the simple truth of how to keep your smile healthy.  Pay attention, take notes and you will get an A in Dentistry 101:

Dentistry has changed dramatically over the past few decades.  We have bonding and implants and digital x-rays now.  We have new techniques that are easier, faster and better. 

There are some things that haven't changed - tooth decay and gum disease.  Let me tell you some basic facts about your teeth:


1.  Nature usually provides us with strong healthy teeth.  They can last a lifetime!
2.  Bacteria grow on teeth.  Unless the bacteria are removed every day, they grow and develop into plaque.
3.  The bacteria eat what we eat.  When we feed them sugar, they produce acid.  Sugar is their favorite food!
4.  If the bacteria and acid remain on our teeth, they eat holes in our teeth.  These holes are cavities.
5.  Bacteria also produce many poisonous substances that cause our gums to become inflamed.
6.  The inflammatory substances are partly responsible for gum disease (pyorrhea or periodontitis).
7.  If you remove the bacteria from your teeth EVERY DAY, you will avoid cavities and gum disease. 

These are very simple ideas, but they are true.  The cleaner your teeth are, the healthier they will be.  Most tooth decay and most gum disease can be avoided by keeping your teeth exquisitely clean and avoiding sugar in your diet. 

Dental hygienists remove the bacterial deposits from your teeth.  They scale and polish away the calcified plaque.  This removes some of the causes of gum disease.  Most people need their teeth cleaned every 6 months.  Some people are prone to gum problems and more frequent cleaning. 

So there it is.  Keep your teeth exquisitely clean every day and you will avoid most of the problems with tooth decay and gum disease!

Which Toothbrush Should I Use???

Choose your weapon wisely in your war on cavities and gum disease!  Dr. McAdams is a big fan of any toothbrush that will inspire you to brush twice a day and brush efficiently…but that’s not really a clear guideline is it?  Let’s start with this:

Electric vs. Manual:

Of course you can do a good job of protecting your smile with either option but you may find the electric to be a bit simpler because it will do the cleaning for you…you just have to hold it in the right spot!  Additionally, you may find less risk of causing more gum recession if you properly use an electric vs. the manual option because so much back and forth scrubbing isn’t required.  Another factor to consider when making this choice is cost:  electric brushes are expensive up front compared to manual brushes, and the replacement heads can really add up because your bristles need to be replaced every 3-4 months with either option. 

Choosing between Electric vs. Manual comes down to ease of use vs. cost in our minds.  You really can’t go wrong if you remember the following:

Going Manual?

If you choose to stick with the old trusty manual brush, you’ll want to refresh your technique.  Dr. McAdams recommends brushing for a full 2 minutes and focusing on 1-2 teeth at a time instead of a whole section of teeth to reduce your tendency to overscrub your gums.  You will want to choose a brush head shape that fits your mouth…small mouth?  Small brush head!  Bigger teeth…you get the idea!  Make sure your brush makes it easy to reach all of the corners of your mouth where plaque tends to hide.  Also, don’t forget the bristle type…Dr. McAdams is always saying that medium and hard bristles are better for cleaning grout or shoes than your sensitive teeth and gums…he ALWAYS wants you to use a soft bristled brush!  A final thought about manual brushes…throw them away every 3-4 months when they start to fray, and replace them every time you get sick because your brush is a great place for germs to hang out and get you sick again!

Key points on manual tooth brushes:  Use good technique to avoid gum issues, choose a brush head shape that best fits your mouth, ONLY buy soft bristles and replace it often.

Choosing Electric?

If you go electric, you will find it is easier to brush effectively so avoid the temptation to overscrub.  You bought this awesome tool so let it do the work for you!  Again, you want to choose a brush head that will easily adapt to all areas of your mouth and you definitely want to stick with soft bristles for all of the reasons mentioned above.  Dr. McAdams doesn’t endorse any particular brand of electric brush, but prefers to recommend a couple of cool features if you are looking for something fancy!  Look into one with a timer so you don’t have to guess if you’ve been brushing long enough.  Also, a sensor that reminds you not to press too hard is a very valuable investment. 

Key points on electric brushes:  Let it do the work for you!  Choose an appropriately shaped brush head with SOFT bristles, and consider one with a timer and pressure sensor.

Now you have all of the info you need to make a good decision…pick the one that you will enjoy using and use it twice a day!

Dr.’s McAdams and Gibbons have been studying options for digital dentistry for years and we are proud to say we have invested in a new technology to make our patient visits even more enjoyable.  We now feature a new technology called CEREC. 
 
What is CEREC you might ask, well, it is a digital impression system with an integrated crown milling and finishing system to make it possible to get your new crown made in around 90 minutes.
 
This kind of technology is not a decision to take lightly, Dr. McAdams has been researching this technology since using it in dental school, but he never felt that it was ready until the available materials expanded to include our two favorite all white, all ceramic, metal free materials called E-Max and Zirconia.  These materials can be just as beautiful and potentially more natural looking than the traditional porcelain, and are proven to be just as strong or stronger under biting stresses. 
 
Using this technology means we can scan your impression with a camera wand instead of using the traditional impression materials and while you wait we can use our milling machine to make your crown and send you home with no temporary crowns! 
 
It saves time and lets you skip the second appointment!
 
Is it for every one or every tooth?  No, there are many situations where the traditional methods are still going to be preferable, but it gives us another weapon to do our very best to take care of you and your smile.

Introducing Michael Gibbons, DMD

When we moved into our new location on Scottsdale Road, we were fortunate to have enough space to share the office with our good friend Dr. Michael Gibbons.  As luck would have it, we needed more space at the same time he needed a new address, so we have combined forces to become the dream dental team!  We are taking this opportunity to introduce Dr. Gibbons to our friends and patients and celebrate his recent award!

He was voted Top Dentist in Cosmetic Dentistry for 2015 by Phoenix Magazine!

We are quite proud that the dental community recognizes what we already knew, Dr. Gibbons is one of the very best dentists in town!  Phoenix magazine sends out a random survey to dentists around town once per year and Dr. Gibbons has been recognized as a Top Dentist, each and every time they have voted Dr. Gibbons as the Top Dentist, this year recognizing his exceptional talent for Cosmetic Dentistry!  In fact, he has been recognized in every single issue since they started the Top Dentist survey in 2003!  

He was ALSO voted Top Dentist in Prosthodontics for 2015 by Phoenix Magazine!

Congratulations again Dr. Gibbons, but this brings up a very important question…what exactly is a Prosthodontist?

In dentistry, you can become a general dentist…or continue your training to become a dental specialist.  Part of what makes Dr. Gibbons so special is in fact his specialty training in the field of Prosthodontics.  Prosthodontics is one of the 9 dental specialties recognized by the American Dental Association and are best described as experts in the replacement and restoration of teeth.  In simpler terms, it is extra training to make you a super dentist!

By now we hope you've heard that we moved to a new office location in 2015!  Its amazing that our need for a larger space brought us together with one of the most incredible dentists in Arizona (or anywhere really!) who had a similar need for a new place to practice.  Dr. McAdams is so happy to announce that we have joined forces with Dr. Michael Gibbons and his awesome team from Scottsdale Prosthodontists!   

This collaboration allows our already amazing team to offer an even more comprehensive approach to dental care by adding a dental specialist, a prosthodontist, to our team!  Dr. Gibbons is  a prosthodontist which means he is a specialist in aesthetic and reconstructive dentistry who can do anything from improve your smile with a cleaning and checkup, to comprehensive reconstructive treatment which might improve your daily life if you have a developmental defect or require repair from a history of oral cancer or traumatic accident.

Dr. McAdams is mostly excited that he has a new partner to keep his ever curious mind engaged and someone to show him the tricks learned over Dr. Gibbons 40 years of experience as a dental specialist.  Also, Dr. Gibbons had a great team who came over to complement our already dynamic team.   Our biggest growing pain is having so much awesome to try to contain...we are sure you are going to agree.

Attention All McAdams Dental Patients!!!

We are proud to announce that we are moving to a newer, bigger, more amazing location near the intersection of Scottsdale Road and Shea Blvd!  Its been a long time coming, but we finally outgrew our little office at Tatum and Shea!  If you are driving near Macayos restaurant on Scottsdale road, keep your eyes peeled for our new sign!  We happily started construction in this location last October and after a whirlwind of construction meetings and weekend site visits we are proud to say we are officially moved as of February 9, 2015.

We know that change can be hard, but you wont believe all of the beautiful additions to our practice, we are so proud to show it off to you and maybe you’ll appreciate our favorite feature...a beautiful kids are where our little ones can be entertained while they wait!

Of course our new equipment will help us keep up our high standards of fast, painless and beautiful dentistry...but you might enjoy our ceiling mounted televisions above the chairs more than that...or maybe you'll appreciate our in house lab which will dramatically cut down on the time it takes to mail your dental work back and forth...gotta love that!

Are You Pulling My Leg?

I’ve been getting a lot of questions lately about a new wave phenomenon called “oil pulling.”  Patients want to know several things about this hot topic, for instance, is it safe for my teeth?  Does it work to improve my oral health?  Are there any risks involved?  What types of conditions can it cure?

After pondering the question internally, I decided I couldn’t really give a good answer until I tried it myself.  This week’s blog will directly address my home made clinical trial and a bit of scientific data I looked up on the web.

To begin I looked up how to do it on the internet, same as you might, and my research tells me that 1 tablespoon of sesame or coconut oil can be used to swish with for a period of 20 minutes or until the liquid becomes a white thick milky consistency at which time it will have become saturated with bacteria. 

I brush my teeth for the directed 2 minutes using my handy electric tooth brush, floss all surfaces of my teeth and I would suggest you do twice daily, and I begin my oil pulling therapy.

As I type this I have a mouth full of coconut oil (since I didn’t have any sesame oil handy), and my first impression is this…it tastes awful.   It is a very bland and naturally oily.  My cursory review of the nutritional labeling tells me that the oil I have chosen has no sodium, no carbohydrates, no protein, no mineral qualities and no nutritional value other than a heavy supply of fats.  Based on this data I will tell you that it seems very safe for your mouth and I do not believe it will cause any harm to your teeth and gums.

At minute 7, I have a strong urge to gag and so I naturally spit out the oil to recover for a moment…whew…close call!  As directed on www.oilpulling.com, I knew this might happen so I am prepared to start again with a fresh mouth full of oil.  Here we go again!

For the remaining 13 minutes of my experiment I search the scientific literature database online called Pub Med to determine the scientific efficiency of this technique.  There is not much research into the topic available to me but I am surprised to learn that one study by dentists in India deemed oil pulling equally as effective as Chlorhexadine (a prescription strength mouthwash which I know to be about equally effective as Listerene in clinical trials) in reducing halitosis (bad breath) and the concentration of microorganisms associated with causing bad breath.  Promising data!  Another study found that oil pulling over 10 days found equal or better reduction in plaque and aerobic microorganisms associated with gingivitis than using Chlorhexadine when both were combined with daily tooth brushing. 

When my 20 minutes has arrived, I am relieved.  My cheeks are sore and my mouth is oily.  I survived my clinical trial with only one mishap along the way, and I can say that my teeth do feel very clean and smooth at this point.  Perhaps this is because they and my tongue are now coated in oil and I believe anything in my mouth would feel smooth at this point.  Looking at the color and consistency of the coconut oil after the therapy, there is no discernable difference in the oil so I can’t say it obviously removed anything from my mouth.  Based on the scientific data I read while swishing, I believe it has. 

I believe that a good mouthwash will help control the concentration of bacteria and other microorganisms in your mouth, especially the ones associated with gingivitis and bad breath.  I know that a 1-minute rinse with either Listerene or Chlorhexadine has the desired effect and now I know that a 20-minute swish with Coconut oil does too.

For my 2 cents, I’d prefer the taste and consistency of Listerene to either Chlorhexadine or Coconut Oil so I wouldn’t risk the potential for gagging or the sore cheek muscles I now sport after 20 minutes of swishing, but if you ask me if it works and is it safe?  I’d have to say it is.  If you want to use oil pulling in lieu of your regular mouthwash, go for it!

Should you substitute oil pulling for your routine home care regimen of brushing and flossing or your routine checkups and cleanings at the dentist?  Absolutely not!  I found no data to support its ability to “cure” dental cavities or toothaches and there is absolutely no ingredient in any oil that can remineralize a cavity as some websites suggest it can.  Believe it or not, you can’t believe everything you read on the internet so you have to find a reliable source and trust your own judgement.

Baby Mama Drama

At McAdams Dental, we have been seeing more than our fair share of pregnant patients lately, maybe it’s just another sign that spring is here or maybe it’s the fact that several of our patients have left sparkling yelp reviews about our meticulous care of their teeth during their pregnancy. Whatever the reason, we are happy to share a bit of information with you regarding our approach to keeping our expecting mommy smiling.

Stay ahead of the game if you can:

It all starts with a good plan, if you are thinking about starting a family or adding another set of baby teeth to your home, you should do your best to minimize the potential for dental emergency during your pregnancy. What a great time to visit your friendly neighborhood dentist for a checkup and perhaps a dental intervention if you need one so that you can delay an emergency dental visit for another 9 months. Dr. McAdams always says a filling now can help avoid root canals and crowns later! 

But what if it’s already too late? Maybe you just found out you are pregnant and now you realize your pesky toothache is getting worse…what to do? A savvy dentist will do everything in his power to delay your emergency dental treatment until your second trimester, or after delivery, however sometimes this is just not an option! 

Pregnant women need to take very good care of their gums!

Studies show that almost 40% of pregnant women have some form of periodontal disease which is a bad statistic when you consider that this type of condition makes you up to 7 times more likely to deliver a pre-term, low birth weight baby! Hormonal changes will make your mouth buildup plaque faster and cause extra irritation in your gums. Excessive gum irritation in some pregnant women can lead to pregnancy tumors on the gums that should be examined by your dentist. This is why we recommend an extra dental cleaning appointment during your pregnancy. You may be surprised to learn that your insurance may cover this extra cleaning…we were! And if even the insurance companies are happy to pay for this, it must be good for you!

Pregnant women also need to take extra care of their teeth!

When you consider how much calcium your baby will need, you can guess that it will come from your hard tissues (bones and teeth) if you aren’t taking in enough in your diet! Moms need to consult with their doctor about appropriate pre-natal vitamin regimens to ensure that you keep your bones and teeth healthy during pregnancy. Also, consider that stomach acids are likely the most damaging liquids your teeth will ever come in contact with. Moms who have morning sickness need to rinse with water after this unpleasant experience, and consider swishing a fluoride containing mouthwash afterwards, instead of brushing your recently softened enamel after vomiting!

Whatever you do, it is best to keep your mouth healthy when pregnant and not let ongoing problems jeopardize the health of your baby. If you need dental treatment during pregnancy, you should find a dentist who you trust to treat you safely and efficiently. At McAdams dental, we use lead shields to protect you from our already low radiation digital x-rays, sensitive formulations of local anesthetic to avoid complications for your little one, and consult with your OBGYN about the most ideal prescription medications for the two of you should it be necessary. We always employ a “least invasive” treatment planning type and do our best to get you comfortable so you can enjoy being an expectant mother with a great big smile!
